  • A screening by HFS (Healthcare Family Services) or a designated screening agency is required and candidates must be found to be in need of nursing facility level of care, and the Supportive Living Program must be appropriate to meet their needs.
  • Candidates will also be required to provide documentation showing that they do not have active tuberculosis.
  • Candidates must not already be participating in another HCBS (Home and Community Based Services) waiver program.
  • Finally, the candidate’s income must be equal to or greater than the current maximum allowable amount of Supplemental Security Income (SSI), and candidates must contribute all but $90 each month toward lodging, meals, and services.
